Location: Philadelphia, PA (Chestnut Hill Area)

Schedule: Monday to Friday, 8:00 AM - 4:30 PM (must be flexible to work beyond these hours as needed)

Responsibilities

• Provide comprehensive home health care to patients in the Chestnut Hill area.
• Manage a caseload of 30 points/visits per week.
• Utilize HomeCare and HomeBase for charting and documentation.
• Perform IV and venipuncture procedures as needed.
• Travel within the service area to provide patient care.
• Maintain strong professional competencies in a home care setting.
• Work weekends, holidays, and on-call shifts as required
  
  

Qualifications

• Minimum 1 year of RN home health experience.
• BSN preferred.
  
  

Licenses/Certifications

• Active RN License in the state of practice.
• Valid Driver’s License.
• Current CPR Certification.
  
  

Skills

• Ability to work weekends, holidays, and on-call shifts as required.
• IV and venipuncture experience preferred.
• Strong professional competencies in a home care setting.
• Proficiency with technology (cell phone, computer).
• Ability to travel within the service area.
