he Registered Nurse (RN) is responsible for providing oversight and management to the provision of service delivery to participants. Provide skilled nursing care, prescribe treatments, and develop appropriate plans of care to participants according to professional nursing standards and primary care orders.

Trinity Health LIFE New Jersey, a member of Trinity Health PACE, is seeking a part time Registered Nurse.  Programs of All-Inclusive Care for the Elderly (PACE) coordinate and provide all needed preventive, primary, acute and long-term care services so that older individuals can continue living in the community. PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today’s fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers and policy makers.

Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

Minimum Qualifications

Must be a graduate of an accredited Nursing Degree program and possess a current, unencumbered, active license to practice as a Registered Nurse in the state of PACE Organization. BSN preferred. Active CPR Certification required.

Minimum of two (2) years of demonstrated successful experience in related services with geriatric population.

Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.

Valid driver's license in good standing and proof of insurance required.

Job Title: Registered Nurse (RN) - PACE Organization

$7,500 Sign on Bonus

Position Purpose:

The Registered Nurse (RN) at the PACE Organization is responsible for providing skilled nursing care, prescribing treatments, and developing care plans for participants. This role oversees service delivery and ensures the coordination of care to optimize participant health and well-being.

What You Will Do:

  • Provide skilled nursing care, including administering treatments and medications as prescribed.
  • Conduct assessments, develop care plans, and collaborate with the interdisciplinary team (IDT) to monitor participant health.
  • Educate participants and families on self-care, medication use, and health management.
  • Ensure adherence to professional nursing standards and regulatory requirements.
  • Serve on committees and participate in the development of Quality Assessment and Process Improvement (QAPI) activities.
  • Maintain accurate and timely documentation of participant care.

Minimum Qualifications:
